Patient Information
As soon as your appointment is scheduled by your surgeon at Volunteer Surgery Center, you will
receive a text, email, and/or phone call with a website link to One Medical Passport . This link is extremely
useful to gather all of the information we must have prior to your surgery date. This web link is easy
to use, and you will be asked for such things as your demographic information (address, phone numbers,
date of birth, etc.).
It is also very important that you answer the questions about your health history while on One Medical
Passport. Please be as accurate as possible so we are fully aware of all of your medical needs.
Please bring your insurance card(s) and photo ID with you so we can make a copy for our records.
If you have an Advanced Directive (Living Will) or a durable power of attorney, please bring a copy with you.
You are responsible for providing an accountable adult to transport you from the facility. Someone should
remain with you for twenty-four hours following surgery (if required by your doctor). You must not operate
any motor vehicle if you have had general anesthesia or intravenous sedation.
Do not bring valuables with you to the surgery center. If you must bring a wallet, purse, or keys, etc., please
leave them in the possession of your driver in the waiting room.
If a minor (under the age of 18 years old) is having a surgical procedure, a PARENT or LEGAL GUARDIAN
must be in the building at all times.
Your physician will provide you with detailed instructions pertaining to the days/hours prior to your surgical
procedure. These instructions must be followed exactly as indicated. Failure to follow these instructions
could cause your procedure to be rescheduled to a later date. (One example of this would be if you eat or
drink the morning of your procedure when your instructions said nothing by mouth after midnight.)
